C. Your FMM Terms of Purchase Agreement.Note: Fill out all the pertinent information required, sign and date forms.

Pack the following into a well protected envelope or package and send to the address below:
- Order Form
- Anti-Piracy Form
- FMM Terms of Purchase Agreement
- Media Master (CD, DVD or CD ROM)
- Artwork layout files (CD)
- Printouts of artwork files
- Payment in full (required - please send a bank check or money order, sorry no personal checks please.) (Credit card? FMM recommends that you send your package in a manner in which you can track it's progress to us: i.e. UPS, Fed-Ex, or US post w/tracking. Please insure your package for it's full value. FMM cannot accept liability for packages damaged in transit.
Your FMM representative will contact you if everything we need to manufacture your project is not included when we receive your package into our facility.
Prepare your design - please review our specifications if supplying your own graphic design or hiring our award-winning design team.
Prepare your audio master - please review our specifications for submitting masters.
Review our Anti-Piracy Policy if ANY of the content used in your project was not created by you.
